Spirit of Snohomish County Awards 2012 Nomination Form:
Youth Category

At United Way’s annual Spirit of Snohomish County event, we continue the tradition of recognizing youth who are truly helping to create opportunities for a better life for all within Snohomish County. The Spirit of Snohomish County Award will be presented to a young person who advances the common good, inspiring those around her/him to serve others.

We are seeking nominations for an individual who:

  • Is a youth leader in the community and has made a significant, exemplary and noteworthy impact in people’s lives.
  • Volunteers or works with any United Way-funded program, initiative or service.
  • Demonstrates a generosity of spirit; he/she truly cares for others.
  • Is aware of his/her public responsibility and inspires others to give, advocate and volunteer.

If you wish to submit a nomination, please fill out the following information. Deadline for nominations is Friday, June 8, 2012.

The winners will be announced at United Way’s Spirit of Snohomish County Breakfast on
Thursday, Sept. 6, 2012.
